Pragmatic Free Trial Meta
Pragmatic Free Trial Meta is a non-commercial open data platform and infrastructure that supports research on pragmatic trials. It is a platform that collects and shares clean trial data and ratings using PRECIS-2, allowing for multiple and diverse meta-epidemiological studies to compare treatment effects estimates across trials with different levels of pragmatism, as well as other design features.
Background
Pragmatic trials provide real-world evidence that can be used to make clinical decisions. However, the use of the term "pragmatic" is not uniform and its definition and assessment requires further clarification. Pragmatic trials should be designed to guide clinical practice and policy decisions, rather than to prove a physiological or clinical hypothesis. A pragmatic trial should try to be as close as it is to real-world clinical practices, including recruitment of participants, setting up, delivery and implementation of interventions, determination and analysis results, as well as primary analysis. This is a key distinction from explanatory trials (as described by Schwartz and Lellouch1) that are designed to provide more thorough confirmation of an idea.
Trials that are truly pragmatic must be careful not to blind patients or the clinicians in order to result in bias in estimates of treatment effects. Pragmatic trials should also seek to recruit patients from a variety of health care settings, to ensure that the results can be applied to the real world.
Additionally studies that are pragmatic should focus on outcomes that are crucial to patients, such as quality of life or functional recovery. Many RCTs that do not meet the requirements for pragmatism but have features that are contrary to pragmatism, have been published in journals of different kinds and incorrectly labeled pragmatic. This can lead to false claims of pragmatism, and the use of the term should be standardized. The development of the PRECIS-2 tool, which offers a standard objective assessment of practical features is a good initial step.
Methods
In a practical study the aim is to inform policy or clinical decisions by demonstrating how an intervention can be integrated into routine care in real-world situations. Explanatory trials test hypotheses regarding the causal-effect relationship in idealized conditions. In this way, pragmatic trials can have lower internal validity than studies that explain and are more susceptible to biases in their design, analysis, and conduct. Despite their limitations, pragmatic research can provide valuable data for making decisions within the healthcare context.
The PRECIS-2 tool evaluates the degree of pragmatism within an RCT by assessing it on 9 domains ranging from 1 (very explicit) to 5 (very pragmatic). In this study, the recruit-ment organization, flexibility in delivery, flexible adherence and follow-up domains scored high scores, however the primary outcome and the method of missing data were below the limit of practicality. This suggests that it is possible to design a trial with excellent pragmatic features without damaging the quality of its outcomes.
It is hard to determine the amount of pragmatism that is present in a trial because pragmatism does not have a binary characteristic. Certain aspects of a study can be more pragmatic than others. Furthermore, pragmatic studies can present challenges in the collection and interpretation of safety data. It is because adverse events are typically self-reported, and therefore are prone to delays, errors or coding differences. Therefore, it is crucial to improve the quality of outcomes ascertainment in these trials, in particular by using national registries rather than relying on participants to report adverse events on the trial's database.

The use of these terms in titles and abstracts could suggest a greater awareness of the importance of pragmatism but it is unclear whether this is reflected in the content of the articles.
Conclusions
In recent years, pragmatic trials are becoming more popular in research as the value of real world evidence is increasingly recognized. They are randomized trials that evaluate real-world care alternatives to new treatments that are being developed. They involve patient populations that are more similar to those who receive treatment in regular care. Pragmatic trials also have advantages, such as the ability to use existing data sources and a higher likelihood of detecting meaningful distinctions from traditional trials. However, they may be prone to limitations that undermine their effectiveness and generalizability. For example, participation rates in some trials might be lower than anticipated due to the healthy-volunteer effect and incentives to pay or compete for participants from other research studies (e.g. industry trials). Many pragmatic trials are also restricted by the need to enroll participants quickly. Practical trials aren't always equipped with controls to ensure that the observed variations aren't due to biases that occur during the trial.
The authors of the Pragmatic Free Trial Meta identified RCTs published up to 2022 that self-described as pragmatic. The PRECIS-2 tool was employed to determine pragmatism. It covers areas like eligibility criteria and flexibility in recruitment as well as adherence to interventions and follow-up. They discovered 14 trials scored highly pragmatic or pragmatic (i.e. scoring 5 or more) in at least one of these domains.
